It's been in the back of my mind for a while to get a pair of Kosmosimas whenever I got into a space sim. This week I decided to start with some flight sims, and to start with one stick. On doing pre-purchase research, though, I read widely that the SCG grip suits small and medium hands. I have large hands: from the top wrist-crease to the tip of my middle finger measures 21.5 cm (8 1/2 inches), and my palm is 89 mm (3.5 inches) wide.
Is the SCG too small for me? I know the MCG suits larger hands, and that will work for my first stick, but for my later left-handed stick I really want a Gunfighter base for a springless throttle quadrant functionality on the X-axis, and the only left-handed stick available is the SCG.

My hand is just about the same size. I have a right handed Gladiator NXT. I completely removed the modular hand rest and it's worked out pretty well. The only part of the grip that feels tight is around my pinky finger. That's exacerbated by using the stick on the table top, above my elbow height, which pushes the bottom of my hand downward. Moving the stick down to where I would mound a gunfighter feels nicer in my hand. I might also be making an effort to avoid inadvertent presses on the pinky button as I move the stick, putting more grip pressure on my ring finger and squeezing my pinky out a bit. Looking really carefully, for my particular hand it would be better if all three finger notches were the same size, rather than having a smaller groove for the pinky, but it's not a "problem" that I notice in game. It sounds like going the other way (small hands on a MCG) causes bigger issues with not being able to reach all the hats without changing your grip. I haven't used one myself.
Besides that, I find it a little awkward to quickly reach around to pull the rapid fire trigger, but pushing it is great. Just don't bind the pull to something you use constantly. Not sure if smaller hands would change that or not.

Using your measurements I'd be 8 1/4in and 4in, my hands are pretty blocky. I actually use the 'palm rest' lifter because I like that hard shelf to push the pinky side of the hand against, and have to arch my pinky finger to use my fingertip on the pinky button (or if I "grip" more naturally, am pushing that button with the inside surface of the hinging point between my last pinky bone and the prior (middle to distal phalanx). Regarding the hat positions the bottom center hat is kind of 'low' vs. my ideal thumb placement, but still reachable. I actually kind of bend the tip of the thumb backwards some (I'm very flexible in finger terms, less so these days in whole body), and kind of stick the middle-to-distal phalanx thumb joint "into" the cup, if that makes sense. The upper hat and ministick are perfectly at my thumbtip, kind of upper-middle of the thumbprint 'bullseye'.
I can easily extend my index finger to almost wrap the upper side of the rapid-fire trigger into the first hinge joint, and I suspect that's slightly abnormal, the shape makes it sufficient to just sort of get the tip barely over the front edge to pull that trigger in.
This is on my LH Kosmo Premium, used laterally most of the time as a throttle replacement, and I'm left hand dominant (my hands appear to be almost exactly symmetrical in size though). I don't think you'll find it too small.
