I have exactly the same material.
In pictures 1 and 3 you can see the part next to the white elongated hole, especially in picture 1.
You should not try to insert the metal tip into this area but into the elongated hole.
It may be that this is just an impression of the photo but I prefer to intervene. Please excuse the translation from French.

In English.
The metal tip must be inserted into the oval hole.
On the photo n°1 posted by Nanashanjin it seems that the parts have been badly adjusted. But I could be wrong.

Yes, the pin is in the deeper hole, Im sure since I keep the stick relatively straight as well.
Biggest problem seems to be, with the extension the forward pressure, tilts the rear part of the extension upwards, hopefully I will be able to tighten the new fasteners a tiny bit more, so the extension will stay in place.

Received the new fasteners!
small note, the new ones didn't have the straight side the previous ones have, they do not fit my stick, no worries I use the new screw in old fastener.
However the round fastener does fit way better into the extension that keeps coming loose.
The screws feel way better, I can definitely tighten them more tight.
Initial response is very good.
Also, with the old fasteners, I noticed loosening the dry clutch helped a bit, but gave the stick more wobble when releasing.
I've tighten the dry clutch again, and do expect the problem to be resolved.
I will let you know if it stays this way!
