I have it bolthead->Cam->washer->locknut->innermounting-ballbearing->nut
For what it's worth, 5 months later, that appears to be correct, although only the y-axis has the flat and lock washers. The x-axis only has a lock washer. It doesn't appear to matter which end the head and nut go on.
