How do you properly attach the universal plate and the Gladiator plate to the UCM-S bracket?

That picture is incorrect. It only comes with 4 nuts which I believe are used for the 4 screws to hold the joystick base. There are two of those large black thumb screw things though. Want to be certain the thumb screw is used for the bottom of the flat screw though and if a washer should be used as well. Also not 100% sure how the universal plate mounts to the UCM itself.
