I tried the linked older version of the bootloader (I think it was the version I tried before) and it gives the same error as shown in the following screenshot:
I ran it as administrator and tried with the stick connected, the stick disconnected and the base disconnected from black box (each time going into bootloader mode by holding the sys button and connecting the USB - so I never ran any version of VKBDevCfg). All attempts produce the same fail message.

I still don't see if you've tried actually resetting the black box. Resetting it basically erases everything and brings it back to it's base state of just the plain jane black box. After doing that you'll see the device shown in the bootloader as something like NJoy32 device. At THAT point you can try flashing a FW. As far as the bootloader goes IIRC different versions of that are supposed to be used with FW of a certain vintage. Newer FW's didn't work with the older versions of bootloader and vice versa. I don't know exactly where that happened. Same rules with the configurator and FW's. You have to look at the top of the configurator window to see how new a FW it's at least tested to work with. Good rule is to not use newer than the one listed. It might work fine, but also might not. The combo I'm using currently is in my sig. I don't have a twist tho.
IIRC you have a >black< black box. I'm not sure of the procedure to reset that one as I have an OG >orange< black box. On mine it's a physical thing. You have to open up the enclosure and short some pins together that say "boot" on the PCB while plugging in the USB. It's really, really no big deal. I've had to do it a couple of times. For all I know the black one is the same. But if that worked it'd get you back to a proper starting point which is where I think you need to be.
If you get it flashed and running in the configurator--hit the default button before doing anything else. Then calibrate. Then proceed.

No I've not tried the hard reset back to shipped firmware yet. I know its a thing but have not been advised to do it yet, so have not. No idea of the procedure with the black black box. It does have a button on the front that I think mirrors pins/jumper on the orange one but the function is not well documented. If you hold that button and plugin the USB it seems to put the black box into boot loader mode (ready for old Z-Bootloader-C to find Njoy32; Z-Bootloader2 can switch the black box to that mode on its own). Don't think it resets the firmware to that shipped. If there is a way to do that with the black black box, I'm guessing might find out soon.

I thought of something. You say it's a regular GF/MCG-Pro stick that you added the twist adapter to. So you're using the twist FW. Try using the non twist FW with it and see if it identifies. I wonder if it's identifying wrong because it's technically a non-twist stick other than the mod.
Reason I'm thinking this is the regular FW has an axis on it already that I think it's for twist. Think it's plain Z axis (that I always disable because I don't have a twist on mine). Another thing worth a shot. I was setting up a plain default buttons profile for MSFS earlier and made me think of it as soon as I disabled that axis.

Thanks @Drano but I have already tried the non-twist firmware, same problem. I am assuming that is when it went wrong though - last time I flashed was when adding the twist module (probably followed some guide that came with it or was available at the time)

You could try to flash any firmware from the MK2 folder. For example, pedal and then roll the right one. Perhaps an iron reset would also help, p.1.3.

Load the attached files in the bootloader; try first with the -1 file, if it works then it's fine. If it doesn't work try the -2 file. Unzip before use.

Loaded up the first file and everything sprang back to life. I have now successfully updated the firmware and reconfigured. If anyone see this in the future, this is the solution.
