All's good, means that the pin wasn't going through the hole all way, so the pins couldn't make contact with the pads. Just press on it when locking the unit.
About the Z axis not centering - first check so the dust cover is not grinding against the grip, then recalibrate.

After a few hours of use it has loosened on its own. could this be a faulty fastener?

You have 2 of them, right? Why won't you swap them, this way we'll know if it's faulty.

Yeah looks like the grip fastener is the thing... the fastener that came with my L is working just fine. The slipping issue transferred immediately with the R fastener.
