NXT EVO not being detected
I have an omnithrottle with an SEM attached. I haven't used it at all except to calibrate it, but when I plugged it in today to try some Elite: Dangerous, it wasn't detected. When I plug it in, the blue light in the base lights up and stays on, and the blue light in the stick turns on for a few seconds then turns back off. I've tried it in different USB ports, and I've tried resetting by using F1+F2 and F1+F2+F3. The former doesn't seem to do anything, and the latter just resets it and does the same thing as unplugging it and plugging it back in, as far as I can tell. No new hardware sound in Windows and VKBDevCfg doesn't see it. I haven't taken the stick off since I installed it, and I used the sleeve, so none of the wires are severed. I'm at a loss here.

Yeah, I should have tried that.
The stick works without the SEM. When plugged in, the SEM will allow me to put it into setup mode, so it doesn't seem to be dead, but plugging it in means neither device is recognized. I don't have an HID interface to test it separately.

Make sure you plug the 3 pin cable into the 4 pin socket on the Gladiator like here.

That's the first thing I checked.
After a little experimentation, it seems that specific combo of BUS3 (the single slot on the left) on the SEM to the vertical EXT2 (the one towards the front of the device next to the encoders) on the EVO is the issue. Plugging into the other 4-pin slots in the SEM from that slot on the EVO, the EVO+SEM combo is recognized and shows up correctly in the VKBDevCfg software, but when it's plugged into BUS3 neither device is recognized. And if I switch the 3-pin connector to the horizontal 4-pin slot next to the 5-pin slot on the left of the base, all three slots on the SEM work. Strange.
(Just for kicks, I put the 3-pin in the 'wrong' way on the EVO and the stick showed up as a KG12. )
So five out of six possible configurations work, but the problem is that I have the SEM on the right and the cable only works in that specific configuration.
I don't know what else to do besides maybe flashing the firmware, but the stick is 2.103 and the newest available is 2.09, so it would be a downgrade and I wouldn't be able to upgrade it again.

I too have a NXT EVO R with SEM on right and have that problem. My installed Firmware is 2.100
I got it to work on day 1 and 2, I had problems on day 3 and since day 4 I can't get the EVO + SEM to be recognized correctly anymore.
I tried to disconnect the SEM but the EVO still wasn't recognized.
Resetting with F1+F2 lets it recognize but POV/Analog mode switch doesn't work anymore.
When I tried to load Default it goes back to not being recognized.
Resetting again.
When I try FIND & AUTO CONFIG it's still recognized and loads the correct default settings but I get the red / blue flashing light.
Tried Calibrating and I think at least the EVO is working again but SEM is now completely useless.
Due to how the SEM is connected and the length of the cable I can't really connect it to any other port.

I did and flashing worked but when I connect the SEM to the EVO again and then plug it back into the USB port it's again not recognized by Windows or the VKB Config Software anymore.

I have the exact same issue my homie, except I'm using the THQ and SEM-V Quadrants. I found that switching the quadrant to the left side ended up working, of course this is useless information if you have the right grip. After brief experimentation I also found that the THQ would work on the right side if you used the 4 to 4 prong to connect the side by side modules, but it only worked momentarily. Every time you turn off your PC the cable must be unplugged and plugged back in which is quite inconvenient indeed. In my case the two solutions only worked with just the joystick and THQ, neither with the SEM so I'm yet to find a viable solution.
Currently sitting here with 400 dollar paper weights, I don't know what I'm gonna do ya'll.
