Ok. Ran the sim with joy2key disabled (I had nothing linked to the Gunfighter) and pedals and throttle unplugged.
Same result. Shifted keys are dead.

You're doing something wrong when binding the controls in game. Maybe you're binding joystick buttons to controls that are not available for certain planes, like pitch and roll trim on german planes?
Try to bind the shifted trigger button to fire guns, take a mission in mid flight and press the trigger. Does it work?

The Bf110 and Fw190 use a rear stabilizer trim and I had those on the shifted buttons. They worked flawlessly up until 2 days ago when the joystick was acting up and I had to reflash in Black Box (Orange actually) to get some of the functions back.
I've tried flashing it as a black black box and an orange black box and followed both your written and video instructions and can not get them to work.
Yes, the trigger works with pinkie shift.

Sit in the cockpit of a Bf110 and look at your left elbow. That big wheel is the elevator trim wheel. To the rear of that trim wheel is a scale that tells you the amount of elevator trim you have dialed in.

Alright, I'll try to replicate your issue. Which profile are you using now - the one that I've made or the one Ron suggested? Also, tell me exactly which button you assigned to which control in game. I'll try to do the same to see what's not working.

I'm using the profile you provided with modifications suggested by Ron.
Pinkie - Shift
Trigger - fire all guns/fire 2nd gun group
Top button - bomb release/launch rockets
Hat forward - raise flaps/elevator trim down
Hat back - lower flaps/elevator trim up
Hat left - raise & lower gear/blank
Hat right - blank/blank

I've been awake since 0600 yesterday and on this problem since 2200 continuously today (my birthday btw) and nearly at my wits end. If only virpil grips were in stock I think I could solve my problem without being such a nuisance here.

First of all Gordon a sincere Happy Birthday! Sorry you're getting through these issues just today :(
And just to be clear about it - you don't have an issue with the joystick, your problem is with the in-game binding. As soon as the joystick works well in tester then the game should recognize those controls. But between the joystick output and in-game input there are lots of things that could get in the way.
Second, I've tried the profile you indicated and I confirm it works in game for both the regular set and the shifted set. What I think it happens in your case is you double binded some controls. I would suggest to erase ALL your previous bindings and start anew or at least check that your new bindings don't have a small orange box in the binding box indicating a double bind.
Do an easy test: assign each pair of buttons to flaps: first, button 1 (standard) to flaps up and button 14 (shifted) to flaps down, then button 2 (standard) to flaps up and button 13 (shifted) to flaps down and so on for the rest of 4 buttons and their shifted pair

I've switched the elevator trim to an axis on my throttle and it worked. That's not a good place for me and I need the axis back for prop rpm but it proved the elevators, at least on an axis, do work. I reassigned the landing gear to a hat button shift and it did not work. I assign 2nd weapons group to a hat shift and it works. Flaps up and down on shift don't work.
I only had three buttons on joytokey for the vr 3dmitgo mod. I uninstalled joytokey and problem with keys remains. Disconnected vr headset, throttle and pedals problem still there.
I spoke with a couple friends who also have vkb sticks and they aren't experiencing any similar problems.
Tonight I noticed the pitch axis far from normal. I now need to move the stick nearly twice the distance which makes handling difficult. That may be because of the many times I've had to calibrate the stick over the past 36+ hrs.
In any event I'm exhausted and giving up. Thinking about turning this Gunfighter Pro into an art deco table lamb. Maybe paint it pink like the lamp in Christmas Story.

I may have discovered it early since all I do is IL2. Seriously

You should adhere to this rule - if in the configurator in the test (or in Joytester) all buttons work (and combinations), but in the game, then this is a problem on the side of the game. Try to assign these buttons in another game and you will see the result. Try to delete the settings file in the game completely and re-assign.

I agree the problem does seem to lie on the IL2 side of the house. And, OMG how many times I've reassigned buttons!
Let's look at one function -mapping elevator trim. I would like to have it assigned to hat up and down shifted by pinkie. I can map them to my toe brakes, mixture control and even roll axis. But put them on a shifted button they don't work. Nothing works with a shifted button even tho the key mapping in settings shows in input.

I've been experimenting again with buttons assignment in Il-2 and realized that the elevator trim can't be assigned to any kind of buttons (shift activated or not); it won't even work on keyboard inputs. The only way to affect the elevator trim was by assigning it it an axis.
I also assigned all the pairs of standard/shifted buttons to flaps up/ flaps down controls and all the buttons on the joystick works on that, I'm getting visual confirmation in technochat.

I too was instructed to open the base plate of my Gunfighter MkII and hold a short against the boot jumpers. But I'm having a bit of a problem locating boot jumpers on a MkII base.
Any assistance would be greatly appreciated.

I wonder where you got this advice to hard reset your device. I'm sure not from someone from VKB as you're looking for it in the wrong place.
Please explain again your issues and post screenshots with your axes profiles.

Everything was going well and I flew a couple flights trying to adjust the x and y axes as before. Midway through one flight the grip stopped working. That's when I noticed the flashing led. I unplugged the black box and reconnected it to the computer. This had worked before but was only a temporary fix today.
I open config app to check that the key mapping was saved and got the alert msg that the grip was disconnected again. I've taken the grip from the extension and the extension from the base and cleaned contact surfaces and pins before carefully reattaching and I still get the disconnected msg. Went to FAQ and found instructions to short out the pins in the black box while standing on your head, reach around the computer to reinsert the usb and watch the monitor at the same time.

I've now done that four times and the red light is still flashing and Zebootloader says I need to reset my device then disappears in a flash before I can click on the reset button. I deleted the config files and downloaded fresh thinking I may be dealing with a corrupted file at some point. When I open config it says I need to flash firmware and when I try that zebootloader freezes and I need to open task manager to end it.

Is there a way to start over with a clean slate?

Attached is the page from the Config Manual instructing the customer to "Go look for it" in the joystick base.
