So through some troubleshooting over discord, it seems like the issue was that the L extension perhaps wasn't seated fully. Upon removing the extension and mounting the MCGU directly to the base, everything worked. I remounted the extension and things are still working.

I added a photo of the connector to the gallery. From what I could tell both the connectors and wires were properly placed. Still doesn't work

For my sanity's sake, I reinstalled the TMW stick to the TMW base to confirm all the buttons still work, and they do.

So my only conclusion is that there is either something faulty with the adapter or with the software.
