Well I figured it out. The grip handle was close but was not seated properly. After extra careful inspection, I got the grip on with no issue AND it twists. Before I get started could you speculate on the following. I never adjusted or removed anything in the base when I took it apart. Just the base plate and the wire connections. I have an analog multimeter but getting a digital one this week. With my meter I get:

3.4 volts across black-red
2.5 volts across black-white
